Joe, 

As for a survey, I would like to create an initial assessment tool to get a sense of students familiarity and comfort level with online learning and computer based activities.  The survey would consist of 10 -15 multiple choice questions.  As per your suggestions a quiz is a great idea - I am not yet familiar with the Data Based Activity.  What would you suggest as an activity to get the data and present the results in a concise manner?

My  hope is to get a sense of the class as a whole while identifying  students that are unfamiliar with computer based activities.  What is of value in the presentation of data are multiple options to view the information, for example, may want to view all the students that responded D to question 3.  If I assign a point value to each response it is possible to do a quiz and look for my students with the scores below the class average.

Can you be a bit more specific about what you want in a survey?

Of course, there is the single question Choice activity.  You can also use a Quiz as a way to administer a survey.

In Moodle 1.6, there is a Database activity which is so completely configurable that it could also serve as a survey instrument.
