OK--I've verified the problem using some different examples.  <sigh>

I'm testing a fix I wrote for this problem, and am asking a question about it on moodle.org.  Unfortunately, there is no easy way to address this problem (you get different behavior, for example, if a student just submits his answers than if he saves without submitting first, and the fixes for the one conflict with the fixes for the other).

I'll let you know as I get more information.
