hi,

i am trying to fill out the application to offer an online course at lane.
of course, this is not a new course; it has been offered at lane many, many
times. however, each of us faculty is now required to fill out this form any time
we wish to offer an online class. you are on this committee, so you know this.

my question is: there is a guidelines worksheet with a rubric for hybrid courses included
in the 16 page application--is this 
the same guidelines worksheet we are supposed to use to count up our estimated points
for an entirely online course? am i missing a form?
