1- Student name
2- Student L number

3- preference for:

Friday May 11  6-8pm    
or
Saturday May 12   9-11 am

timeline: I'd want to be able to give them the link in a letter that will be mailed April 25.

I'm sure we can do something for you.
It'll be very simple.  Students type their names in a box and choose from one of two radio button choices.  I could even put a timestamp on it.  You'd get a file that looks like this (no db)

Monday test - Joe Smith - 3/21
Monday test - Jill Hansen - 3/22
Friday test - Sandy Wilson - 3/21
Monday test - Mark Jones - 3/25

Nursing Dept is in the need of a way for nursing applicants to "sign up for" one of two available dates to take a proctored essay as part of the nursing application process.  130 applicants  - May 11 - 75 "spots available"   May 12  - 75 spots available.

Can this be done so that the responses are entered in a data base without me getting 130 email messages? This needs to be available for student use May 1-12.
