I'm going to file this as a wait-and-see problem: I'd be grateful if you could send me a message as soon possible if you see it reappear--and if it's at all possible, don't change the grades until you hear back from me. That will allow me to inspect the state of the database and to dump the log when the problem appears. And as always, let me know if you have any questions. A redo gets a 1 or 2, and I always include a note. Has anything been changed recently that would cause this?
