Hi,
I received an email from Joe Escobar to send emails to this address while he is away. I don't know if Joe had been working with the old FPD website on the teach server last week because we suddenly have a problem. Please see emails below for details. Any help you can provide would be greatly appreciated as we have no access to all the faculty applications that have been submitted for short-term leave this month. In addition, we have a deadline of this Thurs for additional applications to be submitted, and faculty cannot access the application right now.

The short-term leave form is still on the teach server, and we're still using it until a new form is ready for the lane server. Suddenly, it is missing. Please see emails below from Jennifer Cook. Any advice?

Good morning all!
 
I received an email from a faculty member trying to complete an online short-term leave application.  She said she couldn't get the form to come up on her screen.
 
I went to the FPD website, clicked on the "online application form..." which links to http://teach.lanecc.edu/fpd/grants/forms/stl/  the error message is as follows:
 
"File not found!


The URL you have loaded has not been found on this server.
Please alert the system administrator if you believe you have reached this in error."
Please let me know what we can do to fix this.  Our October applications are due this Friday.
